- Degree Certificates (Completion Certificates)
- All but Dissertation Certificates
- Period of Enrollment Certificates
- Academic Transcripts
- Academic Transcripts are given in sealed envelopes.

There is no specific form for application. Please use A4 size paper and include the following:
- Name(family name should be capitalized), date of birth
- Program(master’s program, doctoral program, research student)
- Major (DID, DICOS, DICOM)
- Year and month of entrance / year and month of completion
- Type and number of copies needed (J/E)
- Purpose of use
- Telephone number (day time)/ e-mail address

When requesting "academic transcripts", you will need an envelope larger than 120×235 mm. |

- 3 working days for certificates in Japanese
- 7 working days
for certificates in English
- Please apply well ahead of time, taking into account the time needed for the mail to reach you.
